**Studio Waiver & Release of Liability** You, hereby agree and acknowledge the following terms and conditions: 1. **Participation:** I voluntarily agree to participate in activities and classes offered by M Dance Studio. 2. **Risk Acknowledgement:** I understand and acknowledge that participation in dance activities involves inherent risks, including but not limited to: falls, slips, collisions, injuries to myself and others, and damage to property. 3. **Assumption of Risk:** I voluntarily assume all risks associated with participation, regardless of whether such risks are known or unknown to the Studio. 4. **Studio Responsibilities**I understand that the Studio’s responsibility is to provide a safe environment to the extent reasonably possible, including maintaining a clean and orderly space, and providing qualified instructors. However, the Studio is not responsible for injuries sustained due to my own negligence, recklessness, or failure to follow instructions. 5. **Medical Information:** I represent that I am in good health and have no physical limitations that would prevent me from participating in dance activities. I will inform the Studio of any pre-existing medical conditions or injuries. 6. **Release of Liability:** In consideration for being permitted to participate in Studio activities, I hereby release, waive, discharge, and covenant not to sue the Studio, its owners, officers, directors, employees, instructors, and agents, from any and all liability, claims, demands, or causes of action for any loss, damage, injury, or death, whether caused by negligence or otherwise, arising out of or related to my participation in Studio activities.
